School of Health Pre-Enrolment Requirements

In preparation for the start of your course, please refer to the checklist below. Your place on the course is subject to the satisfactory completion of the elements noted on this checklist. These must be completed before your course commences.

It is important that you undertake your DBS and any additional checks as required. Failure to complete these checks will mean you will not be permitted to enrol at the start of your course. This will also delay your student finance.

Please refer to the FAQ guidance for important information about the required checks for the course. You should use the checklist below as a tool to ensure you have completed all elements required.

DBS Checklist

Self-Declaration Form

Shortly following acceptance of your offer, you will receive an email containing the link to the online declaration form. If you have not received the link, please email DBS@leedsbeckett.ac.uk as soon as possible. Failure to complete your self-declaration form will delay your enrolment and student finance.

You will be asked to identify on the self-declaration form if you have any cautions/convictions that would not be filtered out on a DBS guidance. Check the details on filtering. Please contact the DBS Team if you require further information/guidance on what should be declared. Any relevant offences will go through a risk assessment process in line with the University Safeguarding Policy.

The form will also ask if you resided outside of the UK after the age of 16 (for 12 months or more), to establish if an Overseas Police Record check may be required.

Please Note: You need to disclose full and accurate information. Failure to do so may jeopardise your offer and subsequent place on the course.

  1. Enhanced Disclosure and Barring Service

    A DBS Check has been identified as an entry requirement for the following Health and Social Care Courses:

    • Adult Health Nursing
    • Mental Health Nursing
    • Physiotherapy
    • Occupational Therapy
    • Dietetics
    • Speech and Language Therapy
    • Nutrition
    • Social Work
    • Sport and Exercise Therapy
    • Counselling
    • Playwork
    • Youth and Community

    If you have accepted an offer onto one of these courses, you will be sent an email with guidance on how to apply, including a link to the online application and details on how to log in. DBS application are made using our secure online system, Atlantic Data Ltd. The email will be automated, and will be sent from dbs@leedsbeckett.disclosures.ac.uk. (Please check your junk mail folder if you cannot see this email in your inbox).

    The Disclosure and Barring Service (DBS) is an executive agency of the Home Office. Its primary purpose is to help organisations to make safer recruitment decisions. By conducting checks and providing details of criminal records and other information, the DBS will help identify  applicants who may be unsuitable for certain work, especially those involving children (under 18) or other vulnerable members of society (this is also referred to as ‘regulated activity’).

    Please note, that we are unable to accept a DBS certificate that has been issued through another institution or employer, even if you are registered with the Update Service. Please note the University does not use the DBS update service. All applicants must complete a new DBS Check through Leeds Beckett University. Failure to complete your DBS will mean you are not permitted to enrol at the start of your course. This will also delay your student finance. 

     
  2. Occupational Health Questionnaire

    Health Validation Questionnaire

    You are required to complete an Occupational Health Questionnaire as part of condition of entry for enrolment for the following courses:

    • Adult Health Nursing
    • Mental Health Nursing
    • Physiotherapy
    • Occupational Therapy
    • Dietetics
    • Speech and Language Therapy
    • Nutrition
    • Social Work

    The purpose of the questionnaire is to confirm that you are fit for your chosen course and the necessary placements and to consider any adjustments that may be needed to help you during your course. The answers to the questionnaire are confidential and will not be shared with anyone without your consent.
    Before you enrol on the course you will receive an email from our Occupational Health provider, Heales Medical. The email will be sent from the email address noreply@heales.com

    • You will need a copy of your immunisation history to complete the questionnaire; you can get a copy from your GP or from a previous employers' Occupational Health Team, if applicable.
    • Please login to the Heales system using the login details provided in the email and complete the form as soon as possible in order to prevent delays to your enrolment.
    • Please ensure your contact details are correct, especially your mobile number. You are not able to go back and amend questions once you have proceeded.
    • If you are not able to source a copy of your immunisation history, answer no to the questions requesting this information and Occupational Health will do a vaccination check when you start on the course.
    • When you start your course an initial health validation occupational health appointment will be booked for you, which you MUST attend to be cleared as fit for practice.
    • Please note, if you are a student on a Nutrition or Social Work Course, you do not have this health appointment.
    • Dependant on individual immunisation status you may be required to attend a number of follow up appointments for vaccinations or blood tests.
